Care Navigation / Care Coordinators Team
Our Care Navigation / Care Coordinators team help our patients find the best care for their medical needs with the doctors and practice nurses and will do their best to answer any queries that you may have. Sometimes they may have to ask you for further details in order to assess how best to help you. Please help them by being as concise as you can about your requirements. All our surgery staff will respect your privacy and are bound by the same rules of confidentiality as the doctors and nurses.
Medical Secretaries
Our secretaries provide secretarial back-up for the doctors. Our secretaries are happy to speak to you if you have a query on a hospital referral. We advise patients to contact us if they have been referred to hospital and have not heard from the hospital three weeks after seeing the GP.
Community Nurses
Our District Nurses and Community Nurses are branches of the nursing profession offering healthcare services to housebound patients who cannot attend the surgery but need nursing care and for whom admission to hospital is not necessary or desirable.
Contact with the District Nurses is via the GP or Practice Nurse.
Health Visitors
Our health visitors are fully qualified nurses with special training in family health. They offer advice, a home visiting service and hold a weekly baby clinic on a Wednesday morning. Health visitors may be contacted on 01895 823956.
Community Midwife
Our community midwife provides full maternity care for our patients in conjunction with the doctors. She also runs antenatal clinics. She may be contacted via the Hillingdon Hospital switchboard on 01895 238282.
